The Fakenham Medical Practice


The Fakenham Medical Practice is a nine-doctor dispensing practice providing care to around 15,000 patients in and around Fakenham in North Norfolk.

We work from modern, purpose-built premises. The partners and staff have a shared belief in the delivery of high quality, traditional personal family healthcare. Patients' interests are kept very much to the fore and there is a strong commitment to develop patient services.

To promote a multi-disciplinary team approach, we share our premises with other providers including a pharmacy, adult social services, and local community services e.g., physiotherapy and midwifery.

The practice is part of Norfolk and Waveney Integrated Care Board and has a branch surgery at Walsingham. We are a training practice for GP Trainees.

You may be required to work across the NNPC, NN1, which includes Wells, Fakenham, Sheringham and Holt.

Main purpose of role

  • To work in accordance with practice guidelines and protocols to provide administrative support and carryout healthcare tasks to meet the needs of the patient.
  • To project a positive, friendly, and professional image of the practice to patients, carers, and other visitors to the practice at all times.
  • To undertake a variety of tasks including administrative and healthcare assistant duties that contribute to the smooth running of the practice.
  • To facilitate efficient and effective communication within the practice and to patients, carers, providers, other healthcare professionals, suppliers, customers and other third parties.

Partners, salaried doctors, trainees, Operations Manager, Nurse Manager, Practice Manager, HCA Manager, Nurses, HCAs, all other Fakenham Medical Practice staff, patients, carers, Patient Participation Group members and other community providers.


Job Duties and Responsibilities


Your main place of work will be at Fakenham Medical Practice you may be required to work within the NN1 Practices.


  • Act as a point of contact between the practice, patients and carers and other agencies.
  • Liaise with HCAs, Nurses GPs, and practice teams to identify patients who have long term health needs and support.
  • Complete and action tasks for the administration team ensuring they are actioned appropriately.
  • Complete stocktaking of medical supplies.
  • Support the practice with administrative tasks associated with patient care.
  • Patient data collection through the registration process.
  • Be involved and participate in Group Consultations to help support the care arrangements for patients and longterm conditions.
  • Undertake telephone contact to manage patients on the practices case load following any unplanned hospital admissions where appropriate.
  • Raise awareness of health promotion, screening, and NHS Health checks in the practices.
  • Manage patientinitiated calls to help/signposting etc., ensuring patients are directed to appropriate service.
  • Document and monitor aspects of patient coordination and service delivery, supporting data collection and audit using the practices clinical systems.
  • Support national screening and immunisation programs and encourage uptake.
  • Work closely with and refer to practice social prescribing link workers where a patient is identified as potentially benefitting from this service.
  • Coordinate practice clinics, and contact identified patients with appointments
  • Document and monitor aspects of patient coordination and service delivery, supporting data collection and audit using the practices clinical system
  • Support national screening and Immunisation programs, vaccination clinics and encourage uptake
  • Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact.
  • Complete personalised care and support plans with patients/persons, involving them in the decisionmaking process.
  • Maintain, update, and continuously improve working practices.
  • Undertake new patient health checks, ECGs, phlebotomy, testing of urine samples
  • Prepare other specimens for collection by pathology service
  • Measure, calculate and record patient height, weight, and BMI statistics
  • Measure and record patient blood pressure and pulse data
  • Assist GPs and/or Practice Nurses in clinics as requested, including annual flu clinics
  • Administer dressings
  • Order clinical supplies as required and sign off deliveries when received
  • Monitor and maintain stock levels in consulting rooms, treatments rooms and on trollies
  • Respond promptly and appropriately to patient needs e.g.,
