An exciting oppurtunity has arisen for an experienced Band 6 Nurse or AHP to join the award winning and NICE recognised SCFT Carers Health Team
The Carers Health Team is an innovative service that aims to improve health outcomes for carers. The service provides professional healthcare input for informal (unpaid) carers over the age of 18 across West Sussex.

The role involves lone working, home visiting, carer clinics, health promotion and outreach work (very occasionally this may be either at the weekend or in the evening) including giving presentations and attending multidisciplinary meetings.


This role involves lone working, caseload management and integrated partnership working with the wider SCFT community services, social services and the voluntary sector.

**Whilst the service covers the whole of West Sussex this post will predominantly be based in the North of the county covering areas such as Horsham/Crawley/East Grinstead/Mid-Sussex but with a willingness to visit patients in the coastal region if required, and attend meetings across the county.
