Community Social Worker (Mental Health)

Hybrid

Barnstaple/Ilfracombe (flexible on location)

£36 p/h | Ongoing Temp | Full Time |

Responsibilities:


  • Supporting vulnerable individuals from a range of backgrounds and needs
  • To undertake assessments using a range of health and social care tools
  • To ensure that all necessary communications and referrals are made to relevant agencies
  • To develop and maintain good working relationships with other professionals within the team

Experience required:


  • Experience working in line with the requirements of the Care Act 201
  • Strong and effective communicator
  • Highly personable
  • Committed and passionate about their field
