NURSING ASSOCIATE REPORTS TO:

CLINICAL NURSE LEAD HOURS:
hours per week Job summary: Working under the direct supervision of the Clinical Nurse Lead and strictly in accordance with specific practice guidelines and protocols, the Nursing Associate will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care


Job responsibilities:
Health promotion and screening; including cytology, routine and childhood immunisations. Requesting pathology tests, for example urine culture, swabs. Venepuncture, ECGs and routine observations.

Wound care and removal of sutures/clips. Annual reviews and management of chronic diseases including asthma, COPD, hypertension and diabetes.

Care for individuals with dementia, mental health conditions, and learning disabilities; utilising, supporting and referring to PCN and core community services such as social prescribers, as appropriate.

Travel consultations.

Contraception and sexual health advice. Chaperoning and assisting patients who are being examined by another clinician. Home visits. NHS health checks.

Effectively manage own time, workload and resources. Report any safeguarding issues concerning children or vulnerable adults. Work effectively with individuals in other agencies to meet patients needs. Provide support to trainee Nursing Associates, HCAs and those on learning assignments/ placements as required.

Communication:
Maintain effective communication with staff members, patients, carers and other members of the primary care team. Refer patients directly to other services/agencies as appropriate, to promote integrated and seamless care.

Maintain accurate and complete patient records of patient consultations and enter on to computer systems using agreed templates and read codes.

Communicate clearly and appropriately, adopting the principles of safe practice and assessment of competence. Attend and participate in practice and clinical meetings.


Administration:

Provide a high standard of professional conduct and nursing care at all times in adherence with the NMC Code of Conduct, maintain own registration with the NMC.

Maintain own continuous professional development, undertake annual appraisal and ensure PDP kept up to date, to meet requirements of revalidation, including mandatory updates.

Ability to obtain and document informed consent (either verbal or written). Ensure infection control guidelines are maintained. Ability to monitor and manage maintenance of stock and equipment to include refrigeration protocols and emergency equipment. Confidentially of information gained at work must be preserved at all times.

Work within your clinical remit and ensure patients are directed towards the most appropriate member of the clinical team. Able to utilise a variety of IT packages, such as SystmOne, Notis, Ice, INR Star etc.

Have an understanding of the components and clinical requirements of the Quality and Outcome Framework and National Directed Enhanced Service and Local Enhance Services for Nottinghamshire.

Health & Safety:

The post-holder will implement and lead on a full range of promotion and management their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures.

This will include (but will not be limited to): Using personal security systems within the workplace according to practice guidelines.

Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines.
