Red Bag Initiative


The red bag initiative has been developed in partnership with Sutton Clinical Commissioning Group, Sutton and Merton Community Services, London Ambulance Service and representatives of the care homes.


The initiative helps to provide a prompt, safe and efficient transfer of clinical care when a resident moves between a care home and other clinical settings, such as; hospitals or 'step up' and 'step down' beds.


When a resident becomes unwell and is assessed as needing hospital care, care home staff pack a dedicated Red Bag that contains standardised information about the resident's general health, any existing medical conditions they have, medication they are taking, as well as highlighting the current health concern.

This means that ambulance and hospital staff can determine the treatment a resident needs more effectively.

We are looking for an enthusiastic individual to take up the role as the Red Bag Coordinator.

The post holder will be based in St Helier hospital and will coordinate the red bags for patients coming in to hospital from care homes.

The coordinator will work closely with the acute hospital, Home First+ and Care Home Support team to enable the successful delivery of this pathway.


Main duties of the job:


  • Coordinate the timely flow of the red bag through the patient's journey from admission to discharge working within agreed procedures and protocols.
  • Provide business support, administrative leadership and general office management to deliver a high quality, comprehensive co-ordination service.
  • To b the identified point of contact for issues relating to the red bag pathway and patient journey.
  • Work collaboratively with the multi-disciplinary teams in the proactive management of the red bag pathway from admission to discharge, ensuring the entire pathway is managed smoothly.

Detailed job description and main responsibilities:
Principal Duties


  • Coordinate the movement and transfer of the red bag daily, across the hospital ensuring the high quality management and safe delivery of the red bag at each touch point of the journey
  • Work collaboratively with the multi -professional teams to ensure that roles and responsibilities in relation to the safe management of the red bag are implemented effectively.

  • Promote the awareness of the red bag initiative across the hospital multi-disciplinary team using various communication tools and mediums.
