Description
To support the Upper GI Cancer team in delivering a high standard support to patients with cancer.

This will involve being in daily contact with patients suffering from a range of Upper GI cancers and those with suspected cancer.

The post holder will be expected to support and signpost patients over the telephone and progress their care pathway in liaison with other colleagues and departments in the hospital and in the wider area to include Macmillan services and local cancer support services.

The post holder will also be expected to be involved with audit and Quality Surveillance reviews.

To work as a member of the Upper GI Cancer team providing patient coordination and care, for patients with on the Upper GI Cancer pathway

  • In conjunction with the Upper GI Clinical Nurse Specialist's provide coordination of high quality patient care through ongoing telephonic/ face to face assessment and proactive identification of needs utilising basic clinical knowledge, approved tools and procedures.
  • Collaborate and communicate with the Upper GI Clinical Nurse Specialist's and wider Health Care Team to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.
  • Single point of contact for patient, carers and the health care team. Triaging enquires to the appropriate department within the hospital or community setting. Actively participate in the provision of routine information and advice, for patients and carers over the telephone, liaising with the CNS as appropriate
  • Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audit


We have an exciting opportunity within the Upper GI Cancer speciality service to provide Macmillan Care - Co-ordinator support to our existing Upper GI Cancer clinical team.

Airedale Hospital is a rural District General Hospital within West Yorkshire and covers a population of around 200,000.


The Macmillan Cancer Care Co-ordinator role is an integral part of the Cancer Workforce and involves direct contact with people living with and beyond cancer, to support them throughout their cancer journey.

You will work as part of the Upper GI cancer team and will be a point of contact for patients and carers, supporting and co-ordinating their holistic care needs, providing treatment summaries, providing health and well-being information and support, supporting personalised stratified follow up and helping to promote cancer awareness.

You will work closely with all grades of medical and nursing staff and wider health care teams.

This role is a integral addition to the cancer workforce, it involves competency based practice supported by an established learning and development programme.

PRIMARY DUTIES & AREAS OF RESPONSIBILITY

  • Coordinate care needs of suspected cancer patients
  • Triage incoming calls and initiate appropriate response according to protocols and individual pathways, liaising with the Macmillan Nurse Specialist as appropriate when non routine
  • Utilising good communication skills, basic clinical awareness and appropriate tools, procedures, to ensure efficient case management
  • Make pre planned outbound telephone calls to patients to assess needs, liaise with the CNS or members of the health care team as appropriate for proactive prevention approach to patient care
  • Identify indicators of patient need or change in condition through telephone contact, and respond appropriately as per protocol and /or liaison with CNS
  • Act as patient advocate and facilitator to resolve issues that may be perceived as barriers to care
  • With collaboration from the Macmillan Nurse Specialist, continue to advise patients on individual selfcare management principles and provide consistent planned followup to reinforce and further promote this information
  • Coordinate community resources as needed
  • Refer complex clinical decisions to the Upper GI Clinical Nurse Specialist for assessment and review.
  • Deliver patientcentred, selfmanagement support and education as necessary in conjunction with Macmillan Nurse Specialist
  • Assist in delivering education and support to carers regarding services, equipment etc as required
  • Assist the Upper GI Clinical Nurse Specialist to develop education and information resources for the public and other professionals regarding all aspects of cancer
  • Utilise Information Technology to enter, document and monitor all aspects of patient care needs and service delivery
  • Provide pathway management for patients by monitoring hospital admissions/attendances and new diagnoses of patients, and arrange appropriate outpatient appointments and MDT discussion
  • Take responsibility for the maintenance of accurate paper and electronic information resources, ensuring confidentiality
  • Work as an effective team member, providing support for other members as required to maintain service provision
  • Act as a resource
