Main Responsibilities of a Care Coordinator
- Assist with the implementation and set up of new packages, working closely with all involved in this process
- Oversee all aspects of clients care packages (updating care plans and risk assessments, completing monthly reviews etc).
- Build and maintain professional relationships with clients and carers based on trust, respect and honesty.
- Assist with delivering effective staff inductions including meet and greets and shadow shifts.
- Complete care staff supervisions, spot/meds checks and team meetings.
- Ensure all shifts are filled and plan in advance for any annual leave, being able to cover work in the field if required.
- Report any incidents/accidents or concerns in a timely manner through our internal recording system.
- Participate in the out of hours on call rota ( £10.00 per week day £30.00 per weekend day additional pay).
- Complete any other appropriate duties and responsibilities as requested by the Registered Manager

Experience/Skills Required for this Care Coordinator role:
- Previous experience in a care co ordination role at least one year.
- Knowledge of CQC guidelines and skills for care good practice guidelines
- Able to work with and follow company policy
- Strong communication skills, ability to converse with other professional's
- Good work ethic
- Attention to detail
- Positive attitude and values in line with Yorkshire Homecare
- Desire to succeed/motivation with a 'we can do it' attitude
- Computer literacy experience in use of Microsoft office suite, Care planner and PASS (training will be given)
