An opportunity has arisen to work within the multidisciplinary team at Falmouth Community Hospital as the Progress Co-ordinator. This position is to assist in the discharge planning looking at providing patients with individualised empathetic plans. This is a seven-day service, flexible hours to meet the service requirements. You will need to liaise with patients, carers and members of the multidisciplinary team in planning for discharge.

You will need to be organised, flexible, show attention to detail, with excellent communication and administrative skills.

  • To assist with the efficient day to day running of a busy ward, providing administrative and clerical support to the nursing, therapy and medical staff providing a 7 day service.
  • This post will be required to work some flexible hours to meet the requirements of the post, eg evening meetings with carers and participation in the Progress Coordinators weekend rota.
  • To meet patients and carers discussing consent, care and therapy plans and involving them in care planning for the ward stay and resettlement.
  • To ensure that patient's needs are addressed, based on the care plan and coordinated in the most timely and effective way.
  • To monitor patients progress and report back progress / inform the team or referrer of any problems regarding timing of interventions, including equipment ordering /delivery and organisation of ongoing care. The expected length of stay is anything from a few days up to 21 days.
  • To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards the chosen home environment.
  • Patients are received from a variety of settings/areas Acute SRUs and are discharged to a variety of places including home and residential settings. They may have care packages, daily living equipment, continence and medication supplies, Early Supported Discharge team assistance, etc with each person having their own personal care plan.
