Role summary 1 To provide comprehensive clerical support to busy hospital wards. 2 To retrieve and prepare case notes for wards. 3 To ensure accurate and timely data input onto the hospital PAS system for all admissions, discharges and transfers. 4 To ensure all investigations and inpatient documentation are filed in the case note folder.


Principle Relationships Ward staff, Secretariat staff, Medical Records staff, Patients Key responsibilities 1 Retrieval and preparation of case notes for ward admissions, ensuring all missing case notes are located.

Organisation and priority of duties to ensure all deadlines are met. 2 Process all admissions, discharges and transfers on the PAS (Bed Management) system in real time. Check with patients where necessary and update patient details to ensure data quality is maintained.


3 Access the Summary Care Record (SCR) on the Clinical Spine Application (CSA) via smart card access to trace patient demographic details and NHS numbers when missing for all emergency admissions 4 On discharge file all investigations and inpatient documentation in the case notes in accordance with trust policy, ensuring that the relevant clinicians signature is obtained.

Then deliver case notes to Clinical Coding. 5 Photocopy case notes and other documentation as required.

Sort and distribute incoming mail 6 Process enquiries for information/case notes both face to face and on the telephone using the PAS system, ensuring confidentiality, Caldicott and data protection guidelines are followed.

Assist patients with the completion of health insurance forms and any other queries.

7 Reproduce case notes from microfilm and disc using the CG Gold database 8 Generate lists from the hospital PAS system as required.

9 Provide cover for colleagues in the event of annual leave, sickness and bank holidays.
