Job overview

We have a fantastic opportunity to recruit a fully qualified Advanced Clinical Practitioner (ACP) to join our well-established team of ACP's within Cardiac Surgery.

The Cardiac Surgery ACP team provide ward based clinical care to patients pre and post cardiac surgery, as part of the multi-professional team, working across all the four pillars of Advanced Practice (Health Education England This not only benefits our patients in terms of continuity and quality of care, but provides our patients with more prompt admissions, seamless inpatient care and discharges, thus improving the patient experience and reducing length of stay.


The team are very passionate about providing high standards of care to our patient group and developing the ACP service further.


Our patients are admitted from across the whole of the Wessex area including the Channel Islands for the specialist care and treatment that this tertiary centre provides.

As a result, our Cardiovascular and Thoracic unit has a fantastic, dedicated inter-professional team who fully support the development and training of staff.

Main duties of the job

WHAT YOU'LL DO
To perform at an advanced level within the four broad based competencies of advanced clinical practice (Health Education England 2017)

  • Expert clinical practice
  • Leadership and management, in order to improve and transform services
  • Education and Development of self and others
  • The Advanced Clinical Practitioner will practice independently as a core member of the Medical Team; you will have responsibility for the medical management of a defined patient or client group under guidance of the Consultant responsible for the care episode.
  • You will demonstrate a high level of competence in specific areas of knowledge, clinical expertise, health assessment and clinical decisionmaking.
  • You will provide specialist advice, education and support to patients, carers and other health care professionals.
  • You will practice clinically within agreed care protocols; these will be monitored by the Lead ACP/Medical Lead.
  • You will advise and support the Care Group management team regarding overall service strategy and future development of the service.
  • The post holder will work across professional, departmental and Trust boundaries as required to develop and support patientcentred seamless care.
