Description
The faster diagnosis patient pathway navigator will proactively manage and track patients,
supporting 28 faster diagnosis pathways.
This essential role will support a safe, efficient, supportive service for all patients being investigated
via the breast two week wait pathway and women undergoing invetigations as a result of recall from
breast screening, by ensuring timely investigations, reporting of outcomes and appoinments are
delivered. Escalating any real or anticipated delays which will prevent the National Faster Diagnosis
(FDS) and Cancer Waiting Time (CWT) standards being achieved in accordance with local protocols
In addition they will help support the whole Breast Care team by coordinating and triaging patient
calls to the breast advice and support line and answering and resolving the less complex calls as
appropriate. They will facilitate nurse led nurse led appointments and ensuring all clinic slots are
optimised and waiting lists navigated e.g. Nipple tattooing lists.
Supporting patients with suspected or a confirmed breast cancer diagnosis who require condition
related clinical information and support and to sign post to the appropriate health care professionals
or to pertinent information / support services as appropriate.
Supporting patients who require updates on their previously confirmed diagnosis or to explain breast
standardised clinical procedures pertinent to clinical pathway via the telephone.
Excellent verbal and electronic communication skills essential


Act as a first point of contact for the breast service, acting independently to liaise with the whole breast team and AHP.

Excellent communication skills are essential. Work autonomously within agreed clinic guidelines and patient pathways. Develop a clear understanding and interpretation of the FDS and CWT standards.

Deliver information and ensure care is expedited appropriately and independently through knowledge and experience within scope of practice and professional boundaries.
