Description
We are looking for a passionate, knowledgeable and enthusiastic Discharge Facilitator to join our hospital based community discharge hub.

This post will compliment the existing complex discharge team who are part of our Community Discharge Hub.

This is one of five hubs working across the Harrogate & Rural Alliance (HARA) delivering integrated adult community service within peoples home and also in the Harrogate District Hospital.

Our HARA community hubs bring together health and social care to keep people safe by following the Home First model until the patient no longer meets the criteria to reside in the acute hospital setting.

The Discharge Hub works as one team to minimise duplication and maximise resources to deliver services over 7 days.


Due to the commitment to maintain patient safety by discharging patients at the first opportunity, the Complex Discharge Team are recruiting.

1 Permanent Band 6 part-time post of 22.5 hours

Knowledge of the Mental Capacity Act, current discharge legislation and complex discharge planning is essential. IT skills would be an advantage but professional development in these areas will be supported.
