A SENIOR CHC ADVISOR VACANCY AVAILABLE WITHIN THE SOUTH CUMBRIA & NORTH LANCASHIRE PLACE BASED TEAM

The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.


Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will deputise for the Team Lead and provide specialist advice to the CHC Advisors.

The post holder will have responsibility for the operational and line management duties of the CHC Team in the commissioning of care packages in the community and in residential settings.

The post holder will work closely with the Team Lead CHC to jointly manage the CHC team and workload.


The post holder will work closely with adult social care colleagues and commissioning leads in the ICB and Local Authority in health developments and strategy and will develop close working relationships Local Authority leads.


Currently we are looking for a motivated and experienced RMN, RGN OR ALLIED HEALTH PROFESSIONAL who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).

You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.

Our hours of work are Monday -Friday, 9am -5pm with the opportunity for agile working.

An exciting opportunity has arisen within our Continuing Health Care Teams. Role involves assessing patients for Continuing Care funding (CHC), holding a caseload and reviewing patients as necessary. Good induction and training offered. Friendly Multi-disciplinary Team. The role is varied and involves working closely with Local Authority, Acute Trusts and with providers


You will work within the Mental Capacity Act and gain experience involving Best Interest decisions and Court of Protection work.

Please note that the main headquarters of Lancashire and South Cumbria ICB will move from Chorley House, Leyland to County Hall, Preston in spring/summer 2023.

Main Duties

  • Deputise for the Service Manager/Team Lead CHC and Complex Cases Team Manager and ensure the effective and efficient use of human, physical and financial resources in the absence of the Team Lead
  • Provide cross cover for Deputy Team Lead CHC team
  • Contribute to professional and policy making activities that impact on the team and other health care professionals.
  • Provide leadership and coordination across professional boundaries working closely with other clinicians within the same area of practice.
  • Possess an indepth knowledge of national policy area and changes to policy relating to CHC. Be actively involved in local policy development and implementation of policy changes within CHC.
  • Attend CHC, PHB, and QMG panel as required and make highlevel complex decisions with regard to client placements.
  • Monitor Delayed Discharges of Care and engage in call conferences with other health and social care professionals about delays.
  • Provide specialist expert knowledge of CHC to clinical and Social Care colleagues and safeguarding adults/Mental Capacity Act/ MHA advice and support.
  • Take a lead role in governance within the CHC Team including undertaking audits and providing reports as requested
  • Provide line management for the CHC Team
  • Lead on multidisciplinary team (MDT) meetings/case conferences, for complex CHC cases
  • Lead on and contribute towards evaluation/audit and clinical standard setting with other members of the team.
  • Undertake annual appraisals for identified team members and provide clinical supervision.
  • Be an information asset administrator.
Clinical responsibilities

  • Use advanced clinical assessment skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable planning and treatment of patient care.
  • Manage a small caseload of complex CHC cases
  • Develop and use specialist skills and knowledge to deliver specialised advice and care.
  • Advise on complex cases, supporting the CHC team in the absence of the Team Lea
