Description
Independently carry out assessments as designated by a registered practitioner as part of clinical pathways. Responsible for a patient caseload within relevant settings.

Recognise variances in expected clinical outcomes and to act appropriately by escalating to and seeking guidance from a registered practitioner.

Provide support and assistance to the registered practitioner staff member in the assessment and treatment of complex patients.


Use specialist technical skills and knowledge to provide a flexible, client centred, approach for patients with a variety of physical, psychological and social needs.

Write departmental and implement Trust wide/departmental policies, procedures and guidelines or partake in feedback (depends on grade and role) Maintain a professional appearance in line with the Trust dress code Uphold the Trusts values and behaviours (available on the Trust web site) and behave in a manner fitting with the responsible position of the post holder, maintaining the public confidence Maintain professional and courteous working relationships with staff and respect the equality and diversity of each and every person he/she comes into contact with in the course of his/her business Uphold the privacy and dignity of the patient and respect the equality of patients at all time Recognise and escalate a deteriorating patient and take initial action where appropriate.

Ensure children and adults are protected from abuse and avoidable harm and report any safeguarding concerns in accordance with Trust policies and procedures.

Carry out formal assessments and treatment plans identified on pathway and provide detailed documented and verbal feedback on patients performance.


Work effectively as a member of a team in providing a patient centred program to engage patients in activities to promote independence and well-being Have an understanding of acute conditions & long term conditions and common pathologies in order to assist with patient education.

Have an understanding of common contraindications for treatment e.g following surgery and to be aware when issues require escalation to a senior practitioner.

Ability to identify own limitations and seek advise from a senior registered practitioner when appropriate. Assist in the implementation, evaluation and modification of care plans.


Work effectively as a member of a team to undertake delegated tasks to contribute to the safe and smooth running of the service.

Under the direction of practitioner, undertake the adjustment and alteration of specific items of equipment in line with appropriate health and safety guidelines and role-specific competencies.

Responsible for the assessment and provision of mobility aids and manual handling equipment in line with role-specific competencies. Be in line with role specific Band 4 competencies.

Responsible for planning own day to day allocated workload, resources and priories workload. Work across the whole service to meet the needs of the service.

Responsible for maintaining stock, advising on resources and, when required, the ordering of materials/equipment needed to carry out the function of the service.

Use a range of verbal and non-verbal skills to establish a professional rapport with patients and encourage their interest and participation in interventions by communicating with them in a way that reflects their views, autonomy and culture.

Understand and act appropriately on, written and verbal instruction from a practitioner to carry out specific treatment and interventions. Liaise with other agencies e.g. multidisciplinary team and referral to other specialist services as deemed appropriate. Responsible for the recording of up to date, accurate information concerning any intervention in line with Trust guidelines.

Accurately complete specific referral and assessment forms as required by the needs of the service and patient.

Be proficient in the use of all appropriate electronic systems required to perform role Participate in both service and trust development plans.

Participate in specific projects or departmental working parties as directed by the practitioner Teach and provide supervision to appropriate staff.

As part of the team, incorporate up-to-date techniques and ideas of positive working into your own practice Participate in team audit and data collection and evaluation activities as agreed and directed Critically examine working practices within the department to identify cost improvements and effective measures, develop and implement action plans The post holder will ensure the proper use of the Trusts resources such as stationery, telephone usage, photocopying & other consumables in the course of business, ensuring mínimal waste and mínimal cost Maintain the confidentiality of the Trust in respect of patient/staff information obtained at all times, and use such information only as authorised for specific purposes.
