Our Adults and Integrated Care Service is responsible for a range of services and activities that support people with social care and health needs, including carers.

  • Build on their strengths and assets Our aim is to work with all our health and social care partners as part of a multidisciplinary community team and deliver Practice to be Proud of to support the people of South Tyneside to Live Better Lives. To enable us to do this, we are changing our practice to have conversations to find out what really matters to the people we work with and we are getting to know what there is in our communities, so we can link people with the right level of support. As a key member of the leadership team, Advanced Practitioners are expected to play a leading role, assisting their Manager to undertake reflective supervision and facilitate practice improvement, motivating and inspiring our practitioners to provide the highest standards of practice, that delivers positive outcomes for people, carers' and their families. The postholder will demonstrate an applied knowledge of Adult Social Care legislation including the Care Act, The Mental Health Act, Mental Capacity Act, Human Rights Act and the NHS Continuing Health Care framework, enabling the Advanced Practitioner to pass on professional knowledge to team members and provide advice on complex procedural matters. This will include confidence and competence in decision making, chairing meetings, and a knowledge and breadth of experience in a range of assessments and interventions. Carrying a small protected caseload of complex cases, the postholder will where appropriately cowork some cases as well as managing their own caseload. Provide supervision, coaching and mentoring for team members and students, you will support the team to raise standards of practice and ensure the service delivers a highquality social work service. Advanced practitioners will work closely with the Practice and Assurance Manager and Principal Social Worker focusing upon research and evidence based social work, sharing this learning across the service through a variety of methods including group supervision and action learning sets Requirements 5 Years Employment History 2 Years References Enhanced DBS from the last 12 months Eligibility to work in the UK Social Work England Registration
