Health Care Assistant - Community Integrated Response Team/Home First

This is an exciting opportunity to be part of Wirral's Community Integrated Response Team, where you be supporting local people to remain independent in their own homes, helping them regain confidence and independence so they can reach their full potential.

About the Community Integrated Response Team (CIRT)

CIRT is an integrated team of clinical staff and professionals including Nurses Physiotherapists, Occupational Therapists and Social Workers. The team have a strong focus on admission prevention and early supported discharged from Emergency Department and assessment areas.

One of the fundamental principles of the Healthcare Assistant Role is to support and encourage our patients to become as independent as possible.


Care is provided to patients within their own homes and your daily tasks will be varied and include supporting people with activities of daily living, personal care, moving and handling, prompting medication, meal preparation and social care and support, working with therapists and social workers to make sure people are achieving their recovery goals.


As our patients need care every day, role holders work over a 7-day shift pattern, 8am - 10pm, Monday - Sunday.

A full-time role is 37.5 hours per week, worked over various shift patterns and part time roles are also available.

Our HomeFirst Health Care Assistants assist registered nurses and therapists to implement assessed care needs and helping maintain the patient's rehabilitation and reablement progress.


Their role and the actions they take support admission prevention by responding to incidents of carer breakdown, provide rapid support for individuals to prevent admission to hospital.
