Most Rewarding Part of the Role Overseeing processes and details of how the Team reaches its goal Opportunity to interact and support the staff who make up an integral part of the Team Ability to take certain decisions which impact the Department in a positive way Having patient contact over the phone and making a difference to patients Primary Duties and Areas of Responsibilities Assisting trained nurses in delivering a high standard of holistic nursing care to patients in the home environment, within the dimensions of the post Communicate and liaise effectively with patients, carers and members of the primary health care team including statutory and voluntary organisations Implementing delegated, planned and appropriate aspects of nursing care unsupervised and being responsible for informing the Community Nurse Team Lead/nurse in charge of observations relating to patient care Contributing to the maintenance of the Community Nurse Team agreed philosophy, aims and objectives and participating in team development To follow policies/guidelines determined by the PCT and may be required to comment on future developments.

To participate in surveys/audits as required Responsible for organising planned/delegated work activities within an occasionally unpredictable work environment To accurately record personally generated clinical observations and work records, maintaining the Community Nurse documentation Be prepared to develop new skills appropriate to the development of the Community Nursing service and in maintaining mandatory training requirements Ensuring confidentiality of patients and their care is maintained To assist in the induction of new staff and to demonstrate own activities to new employees and students within the work area Ensure that any complaint made by a patient, carer or visitor is immediately reported to the Community Nurse Team Lead/nurse in charge Key Results Areas Patient comfort and well-being are paramount Achieving agreed standards of care Working as part of and contributing effectively as a valued team member Other objectives to be discussed, set and reviewed at time of annual appraisal Communication and Working Relationships An ability to effectively communicate routine information to patients/carers where co-operation is required and where there may be barriers to understanding.
