Description

We are excited to be launching a new team in Manchester.

A small multi disciplinary team will be established who main focus will be reviewing funded package of care where somebody is living in residential/nursing or supported accommodation.

Using skills in recovery and rehabilitation, they will work with individuals to maximise their independence and improve their quality of life.

The post holder will be based in Central Manchester but will be required to work across the city and conduct placement visits in other boroughs.

The post holder will be an integral member of the multi disciplinary team. The post holder will aim to maximise independence and well being; assist social inclusion and valued community presence.
