We are excited to be able to offer a 20 month fixed term position for a band 7 Parkinson's Disease Specialist Nurse to support the existing service.

The post holder, will have the responsibility for the planning, delivery and evaluation of Parkinson's services within Calderdale and will provide clinical leadership to health and partnership organisations


In collaboration with the multi-disciplinary team, the specialist nurse is responsible for the diagnosis, treatment, and management of adults with Parkinson's disease.

The environment of care includes Community, Primary and Secondary care, including home visits, ward areas and within out-patient departments.


The post holder will deliver a specialist community-based service for patients with Parkinson's Disease (PD) acting as an expert resource in PD by working closely with patients, carers, consultants, general practitioners, nurses' therapists, social services, and other stakeholders across primary and secondary care including Parkinson's UK.


Will aim to minimise the effect of PD on patients' lives by devising and implementing new pathways of care to support crisis intervention, preventing unnecessary A&E attendances, reducing hospital admissions, a reduction in length of stay, the introduction of remote monitoring and the increase of community nurse led clinics.


Will have continuing responsibility for setting and monitoring standards and quality of clinical practice and ongoing supervision of the specialist nurse team.


  • As a clinical specialist, promote teamwork within a multiprofessional environment, demonstrating critical analysis and decisionmaking skills, participating in the delivery of a clinically excellent, high quality Parkinson's service, influencing, and facilitating change within the department and, where appropriate, the organisation
  • Act as a change agent leading the development of clinically effective practice through the effective utilisation and integration of evidence; setting, implementing, and monitoring evidencebased policies, procedures and protocols
  • Be responsible for the diagnosis, treatment, and management of patients with Parkinson's, as inpatients, outpatients and home visits, taking action to ensure appropriate medical management of the patient in accordance with local protocols.
  • Ensure a culture of continuous quality improvement using audit, patient feedback and reflection on practice by self and other members of the team. Participate in local and national clinical trials and research programmes, informing and recruiting patients.


We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients' homes.

We also are incredibly proud to have almost 150 volunteers here at CHFT.


We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

  • Be responsible for ensuring the planning and coordination of the Parkinson's patient journey including the smooth transition of patients to other settings, promoting effective discharge and communication with interdisciplinary, interagency teams as required
  • Be responsible for ensuring coordination of nursing interventions, influencing clinical decisions and monitoring the quality of patient care provided through using expert clinical knowledge relevant to Parkinson's clinical practice, underpinned by theory and experience.
  • Within a multidisciplinary team environment, develop a culture of personcentred care within the Parkinson's service, communicating regularly with patients, relatives and/or carers; promoting a caring environment where equality and diversity issues are respected, and patients are enabled to be partners in their care.
