Description
The social prescriber would provide support and assessment to patients within the PCN as a link worker capacity.

The Social Prescribing role will be an evolving role which will empower people to take control of their health and wellbeing through referral to link workers who give time, focus on what matters to me and take a holistic approach to an individuals health and wellbeing, connecting people with community groups and service for practical and emotional support.

Social Prescribing Link workers will work as part of the Primary Care Networks Multi-disciplinary Group.

Social Prescribing can help PCNs to strengthen community and personal resilience, reduce health inequalities and wellbeing inequalities by addressing wider determinants of health, including debt, poor housing and physical inactivity by helping to support people to increase active involvement with their local diverse communities.

Promoting Social Prescribing within the PCN. Maintain up to date systems. Responsible for maintaining updated and accurate information on clients, referrals and partners. REPORTING To work effectively with the PCN to ensure quality of service provision to patients within the PCN.

Work effectively with the PCN to collect and collate data for monitoring, evaluation and service improvement. REFERRALS Promote Social Prescribing, its role in self-management, addressing health inequalities and the wider determinants of health.

Work in partnership with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health access and outcomes and enables a holistic approach to care.

Be proactive in encouraging equality and inclusion, through self-referrals and connecting with all diverse local communities.


PROVIDE PERSONALISED SUPPORT Meet people on a one-to-one basis, make home visits or telephone assessments where appropriate within organisations policies and procedures.

Be a friendly and engaging source of information about health, wellbeing and prevention approaches. Help patients identify wider issues that impact their health and wellbeing. Work with the patient, their family and carers and consider how they can be supported through social prescribing.

Help maintain and regain independence through living skills, adaptions, enablement approaches and simple safeguards. Work with individual to co-produce a simple personalised support plan to address the persons health and wellbeing needs. Encourage people, their families and carers to provide peer support.

UNDERSTANDING LOCAL COMMUNITY BASED SUPPORT SERVICES To develop and maintain an active knowledge of the third sector offer in the local community.

To network with local third sector organisations and understand the services that they provide.

To source, develop and manage a range of contact details and sources of information about local third sector organisations and services relevant to the local community and to make this accessible to other PCN staff.

To understand and engage with key third sector networks e.g. Cumbria Action for Health Network, Mental Health Provider Forum and Cumbria Arts and Culture Network as appropriate.

COMMUNICATION To develop and maintain an active knowledge of the third sector in the local community. To network with local third sector organisations and understand the service that they provide.

To source, develop and manage a range of contact details and sources of information about local third sector organisations and services relevant to the local community and to make this accessible to other PCN staff.

To have a full understanding of third sector referral pathways and to use and promote this pathway.

Be proactive within the community and patient network. Work collaboratively with all local partners. Communicate effectively to overcome communication barriers with patients. Ability to cope with challenging and stressful situations.
