Lead Complex Case Manager - Learning Disabilities (Band 8C) (Hybrid)
**£381 per day (Inside IR35)
3 Month Contract**
Birmingham, West Midlands

My Client are an NHS Trust based in West Midlands and are seeking a Lead Complex Case Manager - Specialising in LD on an initial 3 month contract basis (37.5 Hours) Paying £381 per day (Inside IR35).


Job Summary

  • The post holder will play a key role in managing the Children's Continuing Care element, working within the remit of the current Children and Young People's Continuing Care National Framework. The post holder will deliver an effective and competent levelof service that is consistently person
- centred focused for children and young people

  • The post holder will play a key role in developing the All Age Continuing Care Team across Childrens and young people, by liaising with other professionals to facilitate collection of written reports to inform the Continuing Care assessment process. Thisinvolves quality assure the completion of holistic and robust continuing care assessments.
  • Working in conjunction with the wider CCG and external stakeholders, to implement effective service pathways, ensuring the delivery of key quality outcomes and strategic objectives.
  • The post holder will play a key role in delivery of person centred assessments in order to identify any unmet care need which may result in eligibility for continuing care, ensuring the National Framework for NHS Continuing care for Children and Young Peopleis implemented and complies accordingly to the principles and processes with onward referral to other agencies where appropriate.
  • An essential element of the role will include seamless working with other professionals and partner agencies in respect of assessing eligibility for Continuing Care and supporting multidisciplinary teams in the assessment process. The role will ensure thatthe provision of care achieves individual outcomes and ensures the delivery of efficient quality packages of care that is aligned to the Personalisation and SEND agenda and supports the transition of young people into the appropriate adult services.
  • The post holder will work in partnership with stakeholders and other professionals across both the public and private sector, education, health and social care partners to ensure a team around the child ethos for Continuing care is met. This would requireliaison with the relevant health professionals, education, children with disability social work teams and other wider members of the team around the family.
  • An essential part of this role is to ensure each package of care through new or review assessments delivers the best possible care for the child, representing value for money within the agreed available funds. A strong business focus as well as clinicalexpertise is a key requirement for this role.
  • Have a very detailed knowledge and understanding of all aspects of continuing care with detailed understanding of the correct use of the national tools (preassessment checklist, Decision Support Tools (DST), (Fast Track).
  • To facilitate and carry out timely reviews of CYP receiving continuing care and to refer on to others as is appropriate.
  • To ensure that the service provided is of high quality and is culturally sensitive, promoting enablement of the child/young person and their family and preventing over
- dependency on services where appropriate.

  • To provide specialist advice and support to the CCG, to facilitate timely and accurate assessment for Continuing Care needs.
  • To provide written and reports regarding the assessed care needs and care provision for Children and Young people with Continuing Care needs; and the communication of outcomes to referrers.
  • Work with team and other colleagues across primary, secondary and community care to ensure that all patient care areas meet agreed standards, taking personal action to redress any problems in conjunction with relevant multidisciplinary teams.
  • To work with the Continuing Care and Young People Lead, other professionals and partner agencies and provide professional and expert advice and training with regards to the children and young people's continuing care process.
  • The post holder will provide a flexible and accessible service that is responsive to the needs of the child, young people, and their family.
  • To support the Continuing Care and Young People Lead in participating in strategic development of the continuing care service and actively contribute to the development of nursing clinical standards, policies and procedures to ensure that implementationof continuing care is sustainable and meets the assessed needs of children and young people.

Experience

  • In depth experience of undertaking Continuing Care Assessments
  • Significant experience of working with children and young people with complex and longterm health needs, disabilities and those with life limited/palliative or end of life
  • Experience of negotiating and liaising with multidisciplinary team.
  • Evidence of undertaking audit and implementing outcomes in a range of settings
  • Ability to demonstrate autonomous practice, initiating and facilitating evidence based practice
  • Commitment to partnership working across education, health, and local authority settings
  • Negotiation and conflict management skills and the ability to influence decisions.
  • Evidence of leadership, management and team development
  • Evidence of professional/clinical and managerial expertise and knowledge acquired through CPD

Qualifications

  • Educated to degree level or equivalent level of experience working in a specialist area
  • Dual registered sick children's nurse/Registered Nurse with a current NMC registration or equivalent
  • Specialist Community Public Health Nursing (SCPHN) registration/qualification
  • Other relevant specialist post registration qualification, experience
