Health Navigators will be responsible for developing an integrated care pathway breaking down the traditional boundaries between primary and secondary care.

Patients with Post-COVID-19 syndrome (often referred to as "Long COVID") are usually referred into the service by primary care and are allocated a Health Navigator upon triage.

The Health Navigator is the main contact point between the service and the patient.

Patients who have post-COVID-19 syndrome often experience physical and emotional distress requiring support from several therapy services, attached to the COVID recovery team.

The Health Navigators role includes case management, supporting the patient through their journey within the service to ensure this is as seamless as possible.

The post holder will work directly and indirectly with patients and their carers.

Thefocus of the role will be to support triage, coordinate referrals to therapies, provide self management support, track and monitor progress and facilitate video group consultations.
