Please refer to the attached job description and person specification for full details of responsibilities.To act as a link professional for complex patients discharged to the community Discharge To Assess Intermediate Care (D2A/ICT) services.
This role includes clinical triage of patients referred into community services The identification, assessment, planning, implementing and evaluating care as a specialist practitioner.
Including the completion of specialist discharge planning and specialist nursing or therapy assessments, trusted assessor assessments, equipment ordering in accordance with local and National guidelines Provide advice and clinical expertise to other colleagues including those from acute trusts, CPFT inpatient units, other CPFT services and nursing and residential homes to expedite discharge from patient pathways and where possible prevent admissions Make recommendations for health care packages for patients and assess the need for referral to other agencies and disciplines as appropriateMore jobs from Cambridgeshire and Peterborough NHS Foundation Trust
