Accountable to:
Service Coordinator for Family Carers Service

Responsible for:

Managing the delivery of direct support to an agreed case load of young people and adults with disabilities and their family carers.


Job Purpose:

At the direction of the Manager/Coordinator work to ensure that the service offers consistently high quality support to all service users and their family carers, using person centred and user led practices to determine individual and family needs.

Specialising in identifying and supporting service users and family carers from 'hard to reach' communities, including older families.

Relationships:

To develop and maintain proactive professional relationships with families and colleagues from across health, education, other social care agencies and the voluntary sector in the West Midlands.

In addition, to work in close partnership with staff of other Midland Mencap services to promote and develop an organisational approach to supporting people.

Key Responsibilities:
Support Services


  • To support people with disabilities and illnesses and their families to access a wide range of services and community amenities.
  • To support citizens and family carers to access/attend appointments and taster sessions.
  • To offer information, advice and guidance on areas that directly affect the individual's life such as health, education, vocation, leisure, benefits, assistive technology.
  • To offer support and supervision to citizens and family carers in line with their planned programme of activities.
  • To manage an agreed case load of individuals, family carers or whole families i.e. citizen and family carer together.
  • To help develop and run peer support groups for both citizens and family carers.
  • To assist in the preparation and delivery of motivational and learning activities.
  • To attend review meetings and participate fully in giving objective feedback regarding the progress made by citizens.
  • To ensure that accurate citizen records are maintained.
  • To liaise with external agencies on behalf of citizens and their family carers where appropriate.
  • To provide support to other individuals and agencies involved with the citizen or their family carers, to include training and advice on a range of disabilities and illnesses.
  • To participate in regular supervision with the line manager.
General


  • Keep appropriate records and provide regular reports on activity levels and outcomes, as agreed with the line management.
  • Carry out all the above with an understanding of and commitment to Equal Opportunities legislation.
  • Actively participate in team meetings and in own supervision, training and appraisal.
  • Act at all times in accordance with organisational policies and procedures and in a way that will enhance the organisation's reputation.
  • Carry out other duties commensurate with the post, as directed by line management.
  • Ensure that appropriate Health and safety procedures are in place to maintain the safety of service users.
  • To be aware of safeguarding policies and procedures and act to safeguard the well-being of citizens and their families at all times.

£10.65 per hour
