Salary:

We are currently recruiting an experienced full-time Care Coordinator.

In the pivotal role of a Care Coordinator, you become an integral link between our care workers, clients, Next of Kin (NOK), and professional counterparts.

The successful Care Coordinator will have:

  • Previous Care Coordination experience
  • Excellent customer service skills
  • Used planning software such as People Planner, CM2000, etc
  • The ability to work well under pressure, good time management skills and use your initiative
  • Have good influencing skills proven ability to convince others and gain their respect.
  • Excellent communication skills reasonable telephone manner and the ability to build relationships, listen and explain things clearly at all levels.
  • A willingness to participate in an oncall system for outofoffice hours on a rota basis
  • Have an excellent working knowledge of Microsoft Office packages

Main duties -

  • Manage the daytoday operations of the care service, ensuring that the highest standards of care are provided to all patients.
  • Develop and maintain staff rotas to ensure adequate coverage for Patient care, activities, and emergencies while optimising staff scheduling.
  • Monitor compliance with regulatory standards, policies, and procedures to ensure a safe and secure patient environment.
  • Collaborate with patients and their families to develop personalised care plans that address their unique needs, preferences, and goals.
  • Train staff and ensure that all staff are adequately trained and supervised to provide the best possible care for each patient.
  • Oversee the safe and accurate administration of medications, ensuring adherence to prescribed schedules and proper documentation.
  • Facilitate access to external support services, healthcare professionals, and specialists as needed to enhance patient's overall quality of life.
