High Intensity Use Lead, Living Well Tameside

What you'll be doing


As a High Intensity Use (HIU) Lead within our Living Well Tameside service, you will be responsible for supporting people who are being discharged from Tameside Hospital back into the Tameside borough after being in hospital due to a mental health condition.


You will connect with people and provide one-to-one coaching to help them navigate their way through the complex health and social care environment, removing the barriers that may impede self-management and helping to embed coping strategies.

What we need from you

  • You will engage with service users to identify needs and appropriate interventions.
  • You will provide onetoone coaching for people going through a difficult time.
  • You will use your creativity to connect with people and help solve problems.
  • You will develop relationships with partnership agencies throughout Tameside.
  • You will enable referrals and links between individuals and service providers.
  • You will provide support, leadership and motivation to our volunteers.
  • You need an enhanced level DBS check for this role
What we will offer you in return

  • An annual salary of £33,820 (NJC SCP 27)
  • A fixedterm, fulltime contract for six months
  • 25 days holiday, and 30 once you've been here five years
