The role of the Children and Young Peoples (C&YP) Continuing Care Administrator is to actively manage the progress of individuals referred to the Childrens Continuing Care Team.
The (C&YP) Continuing Care Administrator function is responsible for coordinating the pathway for all new cases, to ensure that cases are logged correctly and actions required are fully completed within timescale.
This will include supporting the completion of Continuing Care assessments within 42 days of being screened in and ensuring reviews are undertaken in line with the National Framework for Children and Young People's Continuing Care
The role requires excellent communication skills and an ability to manage complex and sensitive information in a compassionate and professional way.

The job holder will carry out any other duties as may reasonably be required by their line manager.
The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the ICB.
Children, young people, and their families at the heart of everything we do and for that reason the post holder will work within the team and partner organisations across Northamptonshire to ensure that commissioned packages of care are safe, high quality and needs led.
- Core Duties and Responsibilities (Key Result Areas)
Ability to provide and receive complex, sensitive,
or contentious information, for example, the post holder is responsible for:
- Liaising with appropriate professionals in scheduling meetings.
- If necessary, communicating effectively to ensure patients and/or their families understand the process and do not feel threatened by the prospect of the assessment process.
- If necessary, managing queries, questions, and advising on the assessment process and the contribution individual stakeholders can make to the overall assessment process.
- Acting as the point of contact for patients and/or their representatives during the assessment process if the Nurse Assessors are unavailable.
- Responsible for providing routine reports.

- NHFT (Mental Health and Provider Services)
- Acute Trusts (mainly NGH and KGH but other trusts are often involved)
- Community hospitals
- Service Users and family members
- GPs
- General public
- Voluntary sector
- Specialist residential care homes and domiciliary care providers
- Local Authorities (mainly Northamptonshire Childrens Trust but other LAs for out of county placements)
- Legal representatives
- Maintain the team's databases of all activity and financial spend to ensure this is accurate, up to date and reflects all current activity of the team. This will include producing reports with update date information for all new referrals and patients due to be reviewed.
- Support the department with collation of data for regular reports on activity and finances to inform the quarterly reports required by the Northamptonshire Integrated Care Board.
- Process invoices received from care providers; this involves checking accuracy of these against agreed spend recorded against each individual on the team's data base.
- Attend monthly finance meetings to ensure that financial spend is effectively tracked and monitored.
- To ensure timely and accurate record keeping.
- Support the development of information for families, parents, carers and professionals regarding the role of the team and the Continuing Care process.
