An exciting opportunity has arisen to join the Frailty Team at Northampton General Hospital as a Clinical Practitioner for the Frailty team.

We are looking for an experienced and highly motivated nurse who is willing to advance their practice in all aspects of the Frailty speciality and Advanced Clinical Practice joining our developing hospital-based team within the acute services.

The role of the Clinical Practitioner includes identifying Frail patients, performing clinical assessments (understanding the physiological impact of ageing) develop plans of care and co-ordinate safe effective discharges to include the use of allied services.


The Frailty team, work in partnership with the emergency department triaging patients ready for assessment the aim to undertake full holistic assessments providing an all-inclusive comprehensive Frailty service.


The post holder will need to enthusiastically undertake a programme through to completion and be responsible for their own continuous practise development (CPD).


The applicant must have great teamwork and excellent communication and listening skills as the post holder will work within a well-established expert team where training and development opportunities are actively encouraged.

The post holder will have the opportunity to support the developing frailty service and be responsible for junior staff and new learners providing clinical supervision.


The post holder will need to be highly skilled with Information Technology sources as the frailty team uses a range of services to collate information, document care and order investigations.

System-one is used to bridge communication over primary and secondary care, so data protection awareness and compliance is essential.


The post holder will need to be very flexible as the role covers a seven-day specialist frailty care service including bank holidays and absence.
