An exciting opportunity has arisen for a Band 3 Health Care Support Worker within the Outreach and Recovery Team covering Merthyr Tydfil and Cynon Valley.


We recognise that recovery from serious and enduring mental illness is best provided in a patient's own home and as a service we aim to support this as an alternative to regular and lengthy hospital admissions.

The Outreach and Recovery Team work with a diversity of patients with varying mental health difficulties and presentations.

The post-holder will assist the Multidisciplinary Team to maximise patient's independent living skills by delivering effective Care and Treatment Plans in the community.


To provide support and time to an allocated group of patients with more complex needs and thus promote their recovery and maintain them in their community environment.

To assist the professionally qualified team members to assess, plan, implement and evaluate care plans.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Cwm Taf Morgannwg University Health Board is part of the NHS Wales family.
