Breast Cancer Pathway Navigator Band 4


An exciting opportunity has arisen for a dynamic team player to join the team of Cancer MDT Coordinators based at the West Middlesex site on a 12 month fixed term contract.

The Patient Pathway Navigator will be responsible for managing patients through their pathway.

This role will comprise of tracking each patient on the cancer pathway from referral to discharge, including expediting appointments/diagnostics, liaising with other providers, liaising with clinicians, MDT coordinators, schedulers and the services to ensure each patient is treated within the timeframes outlined in the cancer waiting times guidance.

The post holder will be patient centric and provide both the clinical teams and the patients with exceptional levels of satisfaction by helping to navigate the patient pathways to ensure we are compliant to cancer standards.

The post holder will work under the direct supervision of the Team Leaders and Cancer Service Management Team.

This post is part of the patient pathway that aims to deliver an accelerated pathway from referral to treatment for suspected cancer patients and the development of the role will be led by the needs of the patient.

The post holder must be adaptable and willing to work flexibly as the service progresses.

  • Ensure patients are tracked and able to access straight to test services following their initial referral
  • Ensure patients are safely tracked through their suspected cancer pathway and support MDT Coordinator (MDTC) when unavailable
  • Support other tumour sites where needed following the direction of management
  • Provide advice and refer on or signpost to other sources of support
  • Maintain accurate written patient records within the Somerset Cancer Registry and other PAS (Patient Administration Systems) which reflect the care given
  • Participate at all MDT Meetings, Pre PTL and Main PTL meetings weekly
  • Maintain visibility
  • Build good working relationships with both internal and external stakeholders
