An exciting opportunity has arisen within the Wirral Community Heart Failure Team.

We are looking for a highly experienced and dynamic Senior Heart Failure Matron to lead the team into the next chapter of is transformational journey.

Following a successful transformation project and investment the Heart Failure Team has grown, working closely with the CVD Rehab team, Community Cardiology and Cardiac Diagnostics, delivering high quality, safe and responsive care to this complex cohort of patients.

The service is committed to working with system partners to improve the lives of people with Heart Failure on the Wirral.

The Senior Heart Failure Matron will continually influence the development of the whole service to ensure it is patient-centred, safe and efficient and in-line with the appropriate standards.

Provide leadership in the field of cardiology and draw on advanced knowledge and exercise professional skills to develop and deliver high quality service to people with heart failure.

The Senior Community Heart Failure Matron has strong values and is driven to consistently deliver within the role to ensure that nursing, quality; patient safety and transformation are delivered to the highest possible standard within the Community Heart Failure Service.

The post holder will also have an advanced clinical role with responsibility for planning, managing and co-ordinating the care of people with highly complex heart failure needs and associated long-term conditions He /she will use case management techniques to reduce unplanned hospital admissions caused by poor chronic disease management and frailty.

Facilitate early discharge to reduce lengths of stay in secondary care.

Provide expert professional advice and education to patients, carers and colleagues.

Support the Senior Management Team in achieving the Trust's objectives, including clinical governance, controls assurance, risk management, performance monitoring and capability planning.

The post-holder will have line management responsibility for the Band 6 & Band 7 practitioners in the community heart failure team.

This is a senior post within the Community Cardiology Service.
