Would you like to be part the new Virtual Ward Team?


We are looking for a dynamic, adaptable and forward-thinking individual to join the team and develop this exciting new service.

Virtual wards allow patients to get the care they need at home safely and conveniently, rather than being in hospital.

They enable more people to receive the acute care, remote monitoring and treatment they need in their own home. They can help prevent admissions or allow for an earlier, supported discharge. Virtual wards also provide additional capacity to hospital beds enabling more patients to receive the timely treatment they need.


You will join the integrated virtual ward team working across Acute and Community settings together to provide a high level of patient care, clinical governance/oversight and support to the entire MDT.


You will work alongside a team of experienced Consultants, Advanced Clinical Practitioners, Clinical Team Leaders, supporting them to deliver excellent patient care across Central Lancashire.


This post would be ideal for someone looking to work in a new and innovative service, to support the expansion and development of new pathways in this integrated virtual ward service.


The post holder will provide administrative support and effective coordination of the Virtual Ward activity ensuring all information is secure and confidentiality of information is maintained at all times.


The role requires the post holder to communicate with patients requiring excellent customer care which may include communication with distressed and anxious patients and relatives, treating them with tact and empathy.

Ensure the professional image of the Trust is maintained at all times.


The role requires the post holder to be in close contact with clinical teams, operational teams, workforce and financial colleagues and other support services being the link between services to support the patient through their virtual ward stay.


If possible, the post holder will work in line with the Trust agile working guidance working flexibly both on site and from home, where practical.


This is a new and developing service so will require the post holder to be proactive, solve problems and use their initiative to rapidly resolve issues.

The post also has potential to develop as the virtual ward service expands its capacity and scope.

  • Supporting the patient journey by providing a link and point of contact. Ensuring queries from patients are responded to in a timely and appropriate way. Ensuring patient alerts are escalated in a timely and appropriate way via Docobo remote monitoring platform.
  • To be responsible for a highquality customer service function in supporting the coordination and ongoing liaison with referrers and relevant others such as radiology and pathology services.
  • Must be a dynamic person with good organisational and communication skills and a can do attitude. Due to the urgent nature of the work it will require a flexible and adaptive response.
  • Ensure accurate and uptodate patient details are maintained on patient information systems such as FLEX and Docobo in line with Trust Information Governance policy.
  • Ensure any confidential and sensitive data is appropriately managed, archived and stored securely.
