Description

Client focused Treat all clients as individuals, respecting equality and diversity whilst taking a flexible and personalised approach to meet their needs.

Work alongside clients to create personalised care support plans.

Facilitating empowerment and independence by providing and discussing information with the client to enable them to make their own informed decisions and address social barriers.

Advocates on behalf of clients where necessary to ensure best outcomes in relation to long term health and wellbeing.

Manages a large caseload, undertaking reviews of client's needs and setting health and wellbeing goals, using a person-centred approach.

Partnerships Work collaboratively with local VCSE, health and social care, and primary care organisations, to develop multidisciplinary teams, to support adults with their health and wellbeing needs.

Establish links and build working relationships with primary care colleagues and other organisations to identify, build on, and support a range of services and community activities for adults, to promote health and wellbeing.

Takes an active role in projects aimed at improving the health and wellbeing outcomes for specific groups within the community.


Work with multi-disciplinary teams and wider partners to increase take up of preventative activities and services e.g., social activities to prevent social isolation.

Build and maintain knowledge on local provision to help promote to clients and others.

Service development Co-ordinate and deliver information sessions, networking events and workshops within the area, to improve awareness of services and increase joint working between VCSE sector organisations, health professionals and community members.

Prepare and deliver presentations to secure support for the aims of the work, and champion the needs of local adults.


Record, monitor, evaluate and analyse work to ensure that targets are being met, information is recorded accurately, and best practice is developed.

Identify gaps in provision and works with VCSE sector colleagues, and grant panels to address these gaps. Communicating and influencing Confidently and competently adapts written and oral communication to meet the needs of the intended audience. Contributes effectively at meetings utilising diplomacy and influencing skills.

Utilises communication skills to effectively review need, identify root cause and promote best outcome.

Uses skills such as motivational interviewing and behaviour change to promote self-management and ownership of own health and wellbeing as appropriate.

Administration Maintains accurate and timely records, using a variety of media and in line with service policy. Produces health and wellbeing information and written materials appropriate to differing audiences.


Develops and maintains a database of up-to-date contacts and information relevant to supporting the needs of local adults, acting as a resource for relevant organisations.

Performance management Contributes to the production of regular reports and case studies as requested by management and commissioners of the service.

Seeks new and innovative approaches to providing high quality, needs led services. Takes responsibility for own continuing professional development.

Embraces change and new challenges with a positive attitude.
