We are delighted to be able to advertise this new opportunity for a Band 4 Therapy Support Worker to join our growing therapy team in our Acute Medical/Care Units.

We are looking for a senior therapy support worker to deliver evidence based multi factorial assessment and intervention as part of the "front door" therapy team.

The AMU/ACUC therapy team aims to discharge patients as appropriate/to commence therapy and establish a therapy plan for those requiring admission.

The Therapy team also has an important role in being proactive in assessment for and referral to appropriate community supports and services to reduce the risk of readmission.

This is an exciting opportunity to be part of a rapidly expanding therapy service at the point of entry to acute care and in addition to the clinical and administrative duties you will also be involved in service development initiatives and planning.

To work as an integral part of a supportive and collaborative MDT.

To include prioritisation and delegation of workload to Band 3 therapy support workers as needed.


To exercise autonomy in caseload management and have a lead role in the delivery of assessment and establishing a therapy/discharge plan.

Perform administrative duties as required.


Demonstrate effective communication and interpersonal skills, both verbal and non-verbal and written when interacting with staff, patients, relatives and carers.

Produce accurate and legible documentation of treatments


Form professional relationships with clients who may need motivating and encouraging, in a way that respects their views, autonomy and culture.

Work flexibly in response to the changing demands of the service.

Manage appropriately in difficult emotional or physical environments e.g. upset patients/relatives and difficult physical environments e.g. home circumstances.

Working base will predominantly be CGH however you will be required to work across sites (CGH/GRH) as needed.

Evidence of initiative and leadership.

Work and maintain judgement under pressure whilst effectively managing time. Demonstrate an ability to act on own initiative when working independently in challenging environments.

Demonstrate they are an enthusiastic and confident practitioner

As part of the trust the Integrated Therapy Team provides assessment and treatment in acute care and you will work as part of this respected team and as a member of the broader and supportive MDT.


KEY RESULT AREAS


To assess patients, identifying patients' problems, potential complications and contraindications and referring back for assessment by a registered practitioner when necessary.

To be able to assess, evaluate, progress and report back and assist the multi disciplinary team (MDT) in identifying and planning safe discharge.

To identify and organise/undertake appropriate follow up treatment within a delegated framework of responsibilities, reporting back to registered staff and referring on to other agencies when necessary.

This will include follow up home visits and post discharge phone calls to help prevent the risks of early discharge exacerbated by functional difficulties and patient and relative/carer anxieties.

To use clinical reasoning skills to agree functional goals and develop and evaluate the treatment plan. To implement and progress treatment programmes in order to promote physical / functional independence.

To include the instructing of patients on functional exercise, the assessment and issuing of equipment for activities of daily living in hospital and in the home.

Assessing and instructing patients in the safe use of mobility aids and equipment for activities of daily living, and practice activities of daily living such as mobility, safety on the stairs, dressing and bathing.

To provide a collaborative assessment and treatment approach to reduce repetition of information and provide a seamless pathway of rehabilitation from assessment through to discharge, with appropriate follow up interventio
