Stroke Assistant Practitioner:

Band 4**
Countywide

30 Hours per week - Permanent

The Integrated Community Neurology and Stroke Service are looking for an enthusiastic, motivated Band 4 to support the team and facilitate the stroke pathway through WHC.

This is a permanent position and will take the role of Stroke Discharge Co-ordinator, and hold their own caseload under the supervision of a qualified therapist.

You will have an assistant practitioner qualification, have experience of working in a community rehabilitation setting, and knowledge of stroke presentation.

You will be working with a supportive multidisciplinary team comprising of Occupational Therapists, Physiotherapists, Rehabilitation Assistants, Speech and Language Therapists and Dietitians working within Wiltshire health and care, but also have close communication with patient flow hub colleagues and external agencies and clinicians.
