Description

To improve the patient journey by supporting the Division ensuring data is accurate and working with the wider administrative and clinical teams to do so.

To identify and correct erroneous patient pathways to ensure patients are safely managed along their pathway

Use multiple IT systems to track patients along their pathway from referral to first definitive treatment and escalate to the line manager any issues that may impact on the achievement of waiting times standards or patient safety

Identify and correct erroneous patient records to ensure accuracy, and that patients are safely managed along their pathway across multiple patient record systems (Hive/ Meditex/Acubase/Spine)

Capture learning and provide feedback to management as required regarding data quality and performance

Prepare a weekly validation output tracker that quantifies the validation performed and demonstrates details such as outcomes, timescales, next steps, actions taken and more

Review clinic letters generated by the Division ensuring that clinic outcomes are correctly entered on patient records and the correct RTT/outcome/procedure codes have been used

Ensure accurate data quality across multiple reports and systems each week

Assist the data and information officer with HFEA related data submission

Extract relevant data from specified systems to populate activity and performance reports each week

This role involves working to improve the patient journey by supporting the division through validation of patient pathways, ensuring data is accurate and working alongside the wider administrative and clinical teams.


Further responsibilities of this role include:
Provide feedback to management regarding the number and source of data quality issues to ensure effective guidance, feedback & training can be offered to the wider team

Provide cross-cover to work flexibly as team to achieve deadlines and coordinate leave and prioritise workload accordingly

Manage own day to day workload, ensuring that priorities are identified and dealt with and that problems are resolved as quickly as possible

Accurate linking of patient pathways sometimes involving complex cross
- directorate patient activity and information of a sensitive nature

Assist with the management of various databases

Locate and retrieve case notes from all areas of the Hospital if required

Assist other team members in their work as required

Assist in the validation training of administrative and clerical staff

Assist in the preparation of presentations and handouts for validation training sessions

Actively participate in appraisal with line manager, identifying training and development required

General office duties as directed by Responsible Managers
