Working Pattern: 8:15 -16:30 / Mon

Fri / 37.5 hrs

Specific Skills, Attributes & Experience Required

Enhanced DBS
Required to visit patients within their homes, increasing independence in ADL assessing & treating cognitive skills visual impartments, complex postural & equipment needs with access to vehicle & double covid jab
Able to interpret & undertake advanced clinical reasoning analysis from complex clinical & non-clinical info provided, make justifiable assessments, contribute to diagnosis & prognosis of patients & recommend best course of therapeutic intervention
To utilize specific neuro therapeutic skills as required facilitating & optimizing the treatment of patients
Able to communicate sensitively & effectively with patients & carers ensure understanding of conditions & gain informed to be responsible for the daily input of clinical data onto the IPM system in line with Trust Policy

Duties:

Specific Skills/Experience/Training Required:
HCPC Registered
Experience with patients with Acute Illness
Experience managing discharge
Community experience with adults is desirable
