**Permanent, Full Time (37 hours per week).
We are looking to recruit a Senior Practitioner to the South locality Adult Social Care teams.

This is a great opportunity to develop your leadership and supervisory skills in a diverse and dynamic locality.

It is an exciting time to be part of a community Social Care team as we embed the locality model of service delivery.


You will support practitioners to:

  • Work with adults from a variety of groups including physical disabilities, mental health, learning disabilities, older adults and carers.
  • Undertake Care Act assessments and planned and unplanned reviews.
  • Undertake s42 (Care Act) safeguarding enquiries.
  • Maximise use of social capital, reablement support and technology enabled care to undertake support conversations, reassessment where required and update support plans.
  • Support & advise practitioners in all aspects of the Adult Social Care function, including Mental Capacity Assessments, Social Supervision, CPA meetings, Direct Payments and close partnership working with Council, Health and Community colleagues
You will have support from your Team Managers & Service Manager.

You must have an interest in and experience of working in a multi-disciplinary context, be able to work quickly and proportionately and have confidence in respect of decision making and positive risk taking.


Your role will involve supervising Social Care Practitioners, Social Workers, improving consistent practice, increasing use of tier 1 and 2 services and overseeing the development of ASYEs & student social workers; being part of the leadership team and undertaking strategic work; and building relationships with internal and external partners.

You will need to be a good communicator, with excellent time management skills.


Working with Senior Practitioner colleagues, the role will work across both South locality teams with a significant function in implementing a shared duty system ensuring a consistent and timely response to the citizens of South Bristol, and the manageable implementation of the locality model.
