Description

The post holder will be responsible for the delivery of safe care to a caseload of patients allocated to the team, including being professionally and legally accountable.

The post holder will also line manage a team of Community Nurses and Health Care Support Workers. They will supervise and appraise staff members and students.

The post holder will also be required to work along side the Clinical Nurse Lead and Integrated Team Manager The post holder must possess excellent leadership and communication skills.

The post holder must be able to travel between sites.
