16+ Personal Advisor - Herefordshire Council - Leaving Care Team - Negotiable Pay Rate
An exciting opportunity has arisen within the Leaving Care Team.

We are looking for 16+ Worker to work with young people who are aged to help them to access education, training, employment, activities, suitable accommodation and to develop skillsthat enables young people to be equipped to be able to live their lives independently.

We are keen to hear from you if you have experience of working with young people in a similar supporting role and have good understanding of issues such as mental health,substance misuse, housing legislation and support and safeguarding.

Good working knowledge of Pathway Planning and the Leaving Care Act 2000 is desirable.
