We are seeking a dedicated Social Worker to join our Wetherby Neighbourhood Care Management Team in Leeds.At our organization, we are committed to providing high-quality care and support to individuals in need, and we believe in the power of collaboration and community engagement.
Responsibilities:
Comprehensive Support:
Provide social work support within the Wetherby Neighbourhood Care Management Team, focusing on the needs of adults in the community.
Multi-Agency Collaboration:
Work closely with multi-agency teams to ensure coordinated and holistic care for individuals, facilitating effective communication and collaboration.
Assessment and Planning:
Conduct thorough assessments of individuals needs, develop personalized care plans, and implement interventions to support their wellbeing and independence.
Advocacy:
Advocate for the rights and interests of service users, ensuring their voices are heard and respected in decision-making processes.
Documentation:
Maintain accurate and up-to-date records of assessments, care plans, and interventions, adhering to relevant policies and procedures.
Requirements:
Qualified Social Worker:
Must hold a qualification in social work and be registered with Social Work England as a Qualified Social Worker (QSW).
Experience:
Previous experience working with adults in a social care setting is essential, with a strong understanding of the complexities and challenges faced by vulnerable individuals.
Multi-Agency Working:
Experience collaborating with multi-agency teams, including health professionals, community services, and voluntary organizations, is highly beneficial.
Driving License:
A valid driving license and access to a car are required for this role, as travel within the community may be necessary.
