Description

The role will include responding to variations in workload as outlined below:

To support the virtual ward to progress the patient's pathway thereby reducing any delays for the patient and working toward the patients Expected Discharge Date.

To follow up and progress actions to deliver the timely onboarding of patients to ensure they begin their journey on the virtual ward in a timely and seamless manner.

To meet the direct and indirect care needs of patients under the delegation and supervision of a Registered Nurse or Therapist.

To ensure the delivery of high standards of patient care at all times, in line with Trust policies and guidelines

To work as an effective member of the multi-disciplinary team.

To work within the boundaries of the role and refer all other matters outside of their scope of practice to the Registered Nurse or Therapist.

To observe high levels of confidentiality at all times

To manage and prioritise own workload within deadlines
- **To record all types of information accurately and have the ability to produce and maintain documents and spread sheets.
