An exciting opportunity has arisen to join our team on ward 109 as a band 3 discharge facilitator.

A requirement for this post would be good communication skills, IT skills and use of computer systems.

If successful you would be supported and offered training to support the role.

Main tasks will involve supporting the nursing team with facilitating the discharge process to achieve effective patient outcomes and experience

Communicating with Multi disciplinary teams and outside agencies linking in with complex discharges

Communicating with patients and their families to ascertain and identify care requirements which may be upon discharge from hospital
