You will be responsible for the processing of referrals to our Hospice services.

You will be the first point of contact and will carry out the initial clinical triage with patients and other professionals.

You will disseminate relevant information to the multi-disciplinary team to support timely and appropriate care.

The role requires organisational skills, flexibility and the ability to work in an unpredictable environment.

Experience managing patients with palliative care needs and a palliative care qualification is advantageous, but we recognise that all experience is transferable and offer the opportunity to develop your knowledge and skills.

Job Type:
Part-time

Part-time hours: 15 per week


Salary:
£31,910.00-£37,691.00 per year
