The Integrated Discharge Team at Basildon Hospital are looking for a new Facilitator to support with discharge planning.


The role involves working within the Integrated Discharge Team offering support to the Assessors within the team undertaking specific duties involving both direct and indirect care as identified by the Assessors.

Facilitators will also support the delivery of safe and effective discharge for patients from the acute trust.
