Information and Assessment Officer - Hackney Council

- 6+ Month Ongoing Contract

Essentials:

Previous experience conducting assessments with Adult Social Care

Job Role:

Post for access and duty
Acting as the first point of contact and reception service for any customers to Hackney Council's Adult Social Care services
To carry out an initial contact assessment to identify the needs of the service user
To provide information, advice or signpost service users to a range of universal services and recommend and set up any telecare support as required based on the outcome of these assessments
