What does the role involve:

Carrying out full risk assessments and a personcentred outcomebased care plan for each new client
Carrying out care visits as part of the position
Complete quality monitoring visits with your clients
Completing regular care plan reviews
Completing regular MAR chart checks, both on eMAR and paper MAR charts.
Rostered paid "On Call" duties, ensuring that our service runs smoothly during out of office hours and that all communications are documented and shared internally
Liaising with other Healthcare professionals such as Social Workers, Occupational Therapists, GP Surgeries etc
Coaching and mentoring new care staff

Our Requirements:

You must be a caring and compassionate person who is looking to deliver outstanding care and work within the team
You must be willing to travel around Waveney
You must the ability to deal with problems and final solutions, and be able to work with a pressured environment ensuring that the team is competent at all times.
You must hold a valid UK driving licence and have you own transport
NVQ level 3 would be desirable but not necessary but you would have to complete whilst on the job
