What does the role involve:
- Carrying out full risk assessments and a personcentred outcomebased care plan for each new client
- Carrying out care visits as part of the position
- Complete quality monitoring visits with your clients
- Completing regular care plan reviews
- Completing regular MAR chart checks, both on eMAR and paper MAR charts.
- Rostered paid "On Call" duties, ensuring that our service runs smoothly during out of office hours and that all communications are documented and shared internally
- Liaising with other Healthcare professionals such as Social Workers, Occupational Therapists, GP Surgeries etc
- Coaching and mentoring new care staff
Our Requirements:
- You must be a caring and compassionate person who is looking to deliver outstanding care and work within the team
- You must be willing to travel around Waveney
- You must the ability to deal with problems and final solutions, and be able to work with a pressured environment ensuring that the team is competent at all times.
- You must hold a valid UK driving licence and have you own transport
- NVQ level 3 would be desirable but not necessary but you would have to complete whilst on the job
