Hours: 18.5 hours per week (to be worked over a minimum of 3 days)

Salary:
£11,261 per annum

Based:
Home, hospital (Lings Bar) and community based

Tenure:
Fixed term until 31st March 2024

We have an exciting opportunity for an empathetic, caring individual to provide a Patients' Representative Service to older patients, based at Lings Bar Hospital.

Do you have the skills to represent, advocate and support patients and their families, enabling them to make informed decisions around a wide variety of issues during their hospital stay and discharge? Would you like a busy role which sees you working in the hospital, liaising with statutory services and other third-party organisations, as well as making home visits across Nottingham City & County to help facilitate discharge and provide post-discharge support.
