Rotherham Doncaster and South Humber NHS Trust are seeking to appoint a Band 6 Mental Health Practitioner to work within the Rotherham Home Treatment Team with cover arrangements also into the Crisis Team.


This is an exciting opportunity to work within an existing Team that not only provides Intensive Home Treatment but also an Early Discharge Pathway from the Acute Inpatient Services.


The post will be within the Borough Wide Services portfolio and the successful person will be required to demonstrate their confidence as a mental health practitioner with excellent and robust assessment and risk management skills and have the ability to manage complex situations and work well under pressure.


The post will involve flexible duty rota covering Home Treatment seven days a week 9am-7pm but also Crisis Teams shifts which are operational throughout a 24 hour period including night shifts.

To provide an intensive service to support patients in the community as an alternative to hospital admission. To provide a clinical service that is service user focussed.

To triage all mental health referrals to the Home Treatment team to determine the priority for service response and the appropriate clinical pathway.

To carry out full assessments of needs including comprehensive risk assessments of persons referred to the service.

To recommend and refer, where treatment/intervention is indicated, to the most appropriate team/ service utilising all available resources.
To identify where appropriate treatment-based interventions for all service users ensuring that best practice is implemented. Where appropriate to participate in Mental Health Act Assessments so that alternatives to formal hospital admission are considered.
To provide cross cover working into the crisis team when required.

To respect the views of service users, carers, families and significant others, and work collaboratively with them when planning care and assisting them to meet their needs.

To be an active member of the multi-disciplinary team and to attend work-led meetings as required.

Ensure all assessment reports are comprehensively documented using the electronic database System One - maintain up to date clinical notes, progress reports and ensure discharge letters are written for all service users within agreed guidelines/policies.
