Best Choice Care has had an Exciting new Opportunity open for An Care Coordinator to support our branch in Hayes.
Driving is an essential part of the role and previous experience in Care will be advantageous. SPONSORSHIP AVAILABLE...Responsibilities:
- Making Rosta's for the care staff
- Create care plans.
- Make recommendations to improve patient care.
- Document patient care using electronic documentation systems.
- Provide followup care to patients.
- Evaluate patients' progress.
- Participate in interdisciplinary team meetings.
- Investigate alternatives for providing care, such as homebased or communitybased care.
- Ensuring all the care calls are covered
- Being on the On call
- Supporting on Weekends
Salary:
£21,000.00-£28,000.00 per year
Schedule:
- 8 hour shift

Greater London:
